JourneyCare (formerly Hospice & Palliative Care of Northeastern Illinois) provides pain and symptom management and end-of-life expertise to people facing serious and/or life limiting illness. We care for the person, not the disease, because that’s what leads to greater quality of life. By listening and honoring each person’s unique needs, values, desires, and circumstances, we make each day the best it can be. For over 30 years, we have accompanied thousands of individuals and families on this journey, as a guide and partner in care. Our team of physicians, nurses, therapists, counselors, social workers and chaplains, along with hundreds of volunteers, continues to be a trusted resource for care and support in the communities we serve. Our mission is to make every moment count for those touched by serious illness and loss. We are there every step of the way for as long as we are needed.
